Meet Me Sheet
A one-page snapshot, so you can be understood before you even say a word.
What is it?
The Meet Me Sheet is a simple, one-page form that a child or vulnerable adult — or someone supporting them — fills in before an appointment. It captures the important things the person they're meeting needs to know: the name they like to be called, their likes and dislikes, how they might behave on the day, and what helps them feel safe and comfortable.
Who is it for?
It's designed for children and vulnerable adults, including those with SEND (Special Educational Needs and Disabilities), autism, anxiety, communication differences, or any additional needs that make new situations harder.
When might you use it?
- Hospital, GP or dental appointments
- School meetings, therapy sessions or assessments
- Any new setting where you'll meet someone for the first time
It's especially useful when you don't want to explain everything in front of the child or vulnerable adult — the sheet says it for you, quietly and clearly, so the appointment can start on the right foot.
